We are offering two new services to our patients, these are the Neighbourhood Access Hub located in Witney and the Early Visiting Service.
Neighbourhood Access Hubs
Patients unable to get an urgent GP appointment at the practice will be offered an appointment at the Neighbourhood Access Hub, which is located on the ground floor of the Windrush Health Centre. Patients are seen by a local GP or nurse with access to their medical records. The aim is to improve access for same-day requests while relieving GP practices of the pressure of providing mainly reactive care to minor illnesses.
More 20 minute appointments will be introduced by the practice for patients with more complex conditions to focus on preventative care. These patients will be seen by their regular GP in their own practice, as continuity of care and proactive treatment for these patients will deliver better outcomes in the long term.
Early Visiting Teams
Early Visiting Teams will assist GPs to respond to requests for urgent same day home visits. The aim is to provide a more responsive service and to visit in a timely manner rather than the patient having to wait until the GP has finished seeing patients at the surgery. By doing this we aim to offer early support so that the patient’s condition does not deteriorate and result in a hospital admission.
This service will work closely with the practice and with colleagues from other community health and social care services, offering greater support to patients in care homes and those who live in more rural areas. The teams will consist of emergency care practitioners who will have remote access to the medical records of the patients they are visiting.
