Learning Disability Health Check

At Eynsham Medical Group we offer annual health checks for patients with learning disabilities. These can be conducted in the practice or at home.

You can start your annual health check by completing your online review here and we will contact you for your examination once we receive your answers. After your health check your doctor will write a care plan with you.
