MEDICATION FOLLOWING AN OUTPATIENT’S APPOINTMENT OR HOSPITAL DISCHARGE

YOU HAVE A HOSPITAL LETTER THAT RECOMMENDS NEW MEDICATION OR A CHANGE OF MEDICATION

OR

YOU HAVE BEEN SEEN AT THE HOSPITAL AND THEY ARE SENDING YOUR GP A REQUEST FOR A PRESCRIPTION

THEN

YOU MAY HAVE TO WAIT 14 DAYS FOR YOUR NEW MEDICATION TO BE PREPARED

IF

THE HOSPITAL REQUIRES YOU TO CHANGE MEDICATION IMMEDIATELY

THEN

YOU WILL BE ISSUED WITH MEDICATION BY THE HOSPITAL UNTIL WE ARE ABLE TO PROVIDE THE REQUIRED MEDICATION
