Regarding certificates or evidence about your COVID vaccination status
We are unable to issue this certificate at the practice.

Proof of your vaccination status will be available in the NHS app from 17th May 2021, and in some of the other apps that provide you with Patient online services.

If you have not used it before, there is a self-registration process within the app to create an “NHS login”. Information on creating a login can be viewed on the NHS login page on the NHS website.
If you already have access/ login details for patient online services, for example “Patient Access”, you may use this.
Alternatively, you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine. The letter may to take another 5 days to reach you, or more if postal services are affected. Please take account of this when making your plans.

Update 24th June 2021
If you had your first AstraZeneca COVID vaccination at Long Hanborough surgery, and you are waiting to be invited for your second, we have some appointments available this Friday 25th June at Long Hanborough Surgery.
Please call Eynsham or Long Hanborough Surgery (see numbers below) to book an appointment. These can also be booked via Patient Access or via the NHS App.
This is the final AstraZeneca COVID vaccination clinic that the Eynsham Medical Group will be hosting, if you are unable to make this date you will need to book an appointment via 119/NHS App for elsewhere.
Update 11th May 2021
This practice is only offering COVID-19 vaccinations to those patients in priority groups 1-9 who have not yet had their first dose or are awaiting a second dose. Please do not contact us about vaccinations unless you are in one of these groups. You can check your eligibility here .
If you are in the priority groups 10 – 12 (people aged 49 down to 18) you will need to book your appointment via the national NHS booking service when your age group becomes eligible. This will allow you to find a suitable date and time at another site including the Kassam Stadium vaccination centre (or other large centre in a neighbouring county) and a participating community pharmacy. Currently, people aged 40 – 49 can book their first dose on the system, together with people in groups 1 – 9.

Over 40s are welcome to book in to one of our AstraZeneca clinics at LH. please check the NHS app regularly for more availability.
If you have recently registered for the app and are unable to view appointments at our surgery for your Covid vaccination, you must make sure you have completed the ID verification step. Without doing this you will not be fully linked to our surgery and therefore unable to book appointments at one of our sites.
The process involves taking a picture of your passport or other suitable identity document and uploading it to the app. You will then need to take a picture of yourself and wait a short time for it to be confirmed. If you are unable to complete this yourself, you will need to present your ID to our reception team and be sent a linkage key. The process is very simple and usually ID’s are confirmed promptly by the NHS app team.
If you were given your first vaccination at the Windrush health centre before the 28th of February and have not either received a text invite to book via a link or phone call by Monday 10th May to book your second vaccine, please contact reception asap. The easiest way to do this is to fill out an eConsult on our website or via the NHS app.

Eynsham Medical Group have opted to continue delivering covid vaccinations to under 50 year olds. If you believe you are in groups 1-10 for vaccinations then we urge you to join the NHS app to register and book your appointment.
Update 12th March 2021
We have sent a text out this week inviting patients to register for the NHS app and book there covid vaccinations online.
Patient Access will continue to operate alongside the NHS app for our patients who currently use it and they will not be affected.
We have clinics at Long Hanborough for Thursday 18th and Friday 19th running all day and any patient in group 7 and above is eligible to book into these appointments. Please note these appointments are for 1st vaccinations so please do not book if you have already had yours.
You can do this via the NHS app which we encourage you to register with. The clinics will be a large scale and we expect it to be very busy. If you are local we encourage you to walk or cycle and ask that you are mindful of the residents surrounding the practice if you need to drive and park your car. The car park will be kept empty for the patients to use on the day.
Update 1st March 2021
We are currently working on vaccinating Group 6 patients. We are continuing to contact eligible patients dependent on delivery of vaccines from the NHS. If you have not yet been contacted, we want to reassure you that you will indeed be contacted in due course and there is no need to contact the practice at this stage.
We understand that some patients may be wondering why others seemingly younger/fitter than them are being vaccinated before them. This is due to the criteria being used by the Government which does not rely solely on age and means those at risk are being called in earlier.
If you are a primary/sole unpaid carer for someone who is at an increased risk of covid we urge you to contact the practice to complete a carer’s form or alternatively download and fill out the Carer Declaration form. Completed forms must be emailed to email@example.com for processing.
We are working on obtaining consent from all patients and hope to streamline this process shortly. If you have not received a message to complete the consent form then please know that it will not effect when you will be contacted for your vaccination.
We ask that only eligible patients book in online, we run regular searches to cancel appointments for anyone who is not eligible. The Government has recently announced that all adults will be vaccinated by the Summer so thank you for working with us towards this goal.

Update 5th February 2021
We are pleased to confirm that by the end of day on the 14th February we will have successfully vaccinated or have given the offer of one to all of our patients in the eligible groups 1-4.
If you are in groups 1-4 and have not yet heard from us, we ask that you are on stand-by in the coming days to be contacted as we will be running clinics every day week commencing 8th February. We are beginning to contact any remaining eligible patients from today (5th February) to fill them.
We are extremely grateful to the volunteers that have come forward to help at the vaccination clinics at our Long Hanborough Surgery and also the Windrush in Witney. You have been invaluable to the smooth running of the clinics. There too many to name you all but again, thank you.
Our clinicians have been working tirelessly to roll out the programme with Dr Latif at the forefront of the operation. We are also grateful for our patients not contacting the practice regarding the vaccination and ask you to continue to avoid calling the practices about it. We will contact all of our eligible patients in good time.
